Bibliography
This is a consolidated bibliography. This is partly because the
Internet's recent time-frame and rapid evolution blurs the
distinction between primary and secondary sources and makes such
distinction unhelpful, and partly because consolidating the
bibliography makes it easier for the reader to refer to the works
of specific authors mentioned in the text. Square brackets ([ ])
denote online sources. In citing electronic sources I have followed
the guidelines suggested in Melvin Page, 'A Brief Citation Guide
